As the editor of one of the oldest and most influential medical journals, Godlee is leading several campaigns to change the way science is reported, including opening up data for other scientists to review, and digging up data from old and abandoned trials for a second look.
She has strong words about the overuse of drugs, and the influence of industry on the types of questions that scientists ask, and the conclusions that are drawn from the evidence. A wide-ranging investigation into generic drug prices took its most significant turn yet on Thursday, as state attorneys general accused two industry leaders, Teva Pharmaceuticals and Mylan, and four smaller companies of engaging in brazen price-fixing schemes - and promised that more charges were coming.
A civil complaint filed by 20 states accuses the companies of conspiring to artificially inflate prices on an antibiotic and a diabetes drug, with executives coordinating through informal industry gatherings and personal calls and text messages. Officials said the case was a small example of broader problems in the drug business. Generic drug makers hoping to begin selling a new drug first seek out rivals, the suit says, in hopes of reaching an agreement on how to maintain market share and avoid competing on price.
A separate anti-trust investigation into Mylan was recently launched in New York over price-fixing on public school EpiPen contracts. If our system wanted to help people improve then our insurance companies and medicare would be about a holistic approach.

Eventually I came across a federal agent who told me that drug companies were routinely overmanufacturing the pills and dumping the excess in Mexico as a charitable tax write off. The practice was halted, but today the drug companies do effectively the same thing with legal imposition of their wares on arrestees and others deemed mentally ill.

These 'harmless' supplements are being doled out with no such rigorous testing at all. If they were trying to help, they would give it to you for free. This is upselling, pure and simple and in the long run, it will devalue the role of a pharmacist. There is absolutely no place for this type of commercial influence in medicine. We should not stand idly by while our patients are being hoodwinked in this way. We will be guilty by association if we do not condemn this underhand, profit driven deception.
If a product genuinely alleviates the side-effects of a prescribed medication, I see nothing wrong with a pharmacist recommending its use, but only when these side-effects are already evident in the "patient" who is asking for relief.
As for recommending a particular brand of the product in this case Blackmores , this is surely unethical. Shame on you, Blackmores and the Pharmacy Guild: I think any such "deal" is outrageous. I, for one, shall not be buying Blackmores' products in future. My wife has had a chronic illness for most of her life and we have been dealing with medics together for a quarter of a century. Doctors are like mechanics. They know a hell of a lot about a hell of a little and anything outside of their scope of knowledge they automatically condemn.
Pharmacists are just as knowledgeable, but in a slightly different stream. Homeopaths, Chiropracters, dieticians, even massage therapists can be useful in returning a person to optimal health. We all have the power to say "no thanks".
If you can resist the jellybeans at the front counter, surely it is easy to say no to these products that only empty your wallet at your expense which you later flush down the toilet. For a significant number of people picking up a script is an extrememly vulnerable moment.
Step in a professional wearing a white coat giving out "advice" and well The pharmacy huil;d cannot expect the public to have any confidence in the advicer of pharmacists, with this, with advice from pharamacists being bought by disreputable companies to push their products drug pushers?
But then, as I am in a state where legislation is passed due to the money paid by foreign multinational corporation paid to members of the government, I supposed that it is not really that surprising. And, in a country where gambling clubs running numbers rackets are given financial assistance by governments,, it is not surprising that the corruption is continually increasing.
A pharmacist is a 5-year trained health professional! No self respecting pharmacist is going to sell something just to make a buck - especially NOT if it may in any way adversely affect the health of the person standing in front of them.
However, if there is good evidence to suggest that a complementary product will help a person, then they should be offering it as an add-on sale - it would be unethical NOT to.
The AMA doesn't have to have a negative comment for everything. Apart from that - just because the Pharmacy Guild recommends that a pharmacist offer an add-on sale does not mean that every pharmacist will blindly do so.
Pharmacists are able to make a professional judgement for every individual person and determine whether or not a complementary product will benefit them.
They are selling all manner of rubbish right now. Magnetic bracelets, patches to 'draw impurities out of your feet', anti-aging creams and more vitamins than you shake a much cheaper basket of fresh fruit and veg at.
A perfectly reasonably comment. But why should a pharmacist be obliged to do this for one brand only under contract? And where is the evidence? This is another example of Doctors claiming to be the only safe, moral health practioners. I think pharmacists will be offended by the insinuation that Pharmacists will automatically try to maximise sales. Q10 will not do harm, but there is insufficent evidence that it has any real beneficial effect.
Most pharmacists are offended by Blackmore's Program, in that it assumes them to be easily led and are interested only in profit.
Pharmacists learn far more Pharmacology the make up and action of drugs in their training than Doctors. The AMA is trying to assert that the high moral ground belongs exclusively to their members. The dedication of Pharmacists to their customers care is of an equally high moral standard.
Since when has scientific evidence been the one and all? Every 6 months science has new findings and often does as full degr spin on their previous recommendations. People like Rosemary have got it sussed, thank heavens. As well as the issues already mentioned, a conflict that stands out to me is that a lot of the medications doctors prescribe are heavily subsidised by the federal government so people can afford them. I am concerned that if if can be documented that people are regularly buying these pharmacist-recommended complementary medications at their own expense, the subsidies might go and we will all be worse off.
Complementary medications are not covered under the PBS - that only covers prescription medications. I mean what would GPs sign their prescriptions with if it was not for the drug companies' pens?!!? Well aren't pharmacies commericial businesses? They have a right to dertermine how they operate their business. Seems the AMA are whingeing again, best they look at their own "morality" first. The AMA complaint is only the huffing and puffing of indignation that doctors were overlooked by Blackmore's for the humble Pharmacist.
It must have been a dagger in the group ego. It is obvious that the AMA feels threatened because many doctors are in cahoots with pharmaceutical companies. It must be either prejudice or ignorance that prompts such an outrageous falsehood. If they took the trouble to do the research they would realise their mistake. I have a doctor who encourages me to use supplements. It is a pity there are not more of them. I understand the AMA is putting pressure on such doctors to desist in this practice.
Since the main difference between a pharmaceutical drug and a natural complementary medicine is that the former works by interfering in bodily processes hence the side effects whilst the latter works by supporting them. I know which I prefer. Pharmacists, and doctors for that matter, already recommend certain supplements with prescription drugs. For instance, statins deplete Coq So it's hard to judge without knowing exactly what products with what drugs.
Pharmacists already promote certain brands as well - generics - as they're more profitable. In the end pharmacists are free agents and will act within their own ethical guidelines. All because a certain brand pops up in dispensing software doesn't mean they will recommend it.
What about MBBS doctors and the pharmaceutical lobby? Glass houses, the first stone blah? I've had to wait 7 weeks to see a cardiologist because he is the preferred by my GP's practice? He only visits the area once a fortnight? The question is "when" will medical doctors join the anti-Labor crusade?
Notice that both doctors and pharmacists receive computer prompts to remind them what to offer you. How much money could the government save by substituting many of these professionals with deregulated retailers, computers that analyse self-reported symptoms, dispense medicines or give advice - surely the vast majority of health consumers are capable of reading and researching themselves?
Only good things can come out of teaching non-retarded people to be more self reliant. Instead, thanks to atavistic attitudes of public dependence and professional mysticism, the tax payer has to cover the rather high wages that bulk billing physicians and pharmaceutical benefit scheme pharmacists earn through government contributions for their almost superfluous role in the healthcare dynamic.
The public mentality is slowly catching up to the reality of today's healthcare practitioners - many of whom are riding the wave of reputation that was build by their professional counterparts in the past, when such professionals had to do real work with their hands to make medicines and fix problems, yet this wave will subside as we realise that today's medicos are merely trying to distract from their partial obsolescence with hyped up trumpeting of 'services' and 'information capital'.
Most supplements are a waste of time unless you are truely deficient in something; however some supplements have been scientifically proven to work, e. Therefore it is appropriate for chemists to recommend complementary medicines as long as they recommend items that have been proven to assist. Blackmore's products have a reputation for quality, I am not sure if it's warranted, or not.
What I know is that they are unreasonably expensive in comparison, also that the company had contributed to John Howard's Liberals via donations. They are off my shopping list for the above reasons. The AMA are afraid that this might be the thin end of the wedge - pharmacists encroaching on "their territory".
The AMA screams blue murder over perceived threats regularly. The AMA is the most powerful trade union in the country, concerned NOT with patients' health but with maintaining the already unethically inflated pay-packets of their protected members.
Any statement from this mob should be treated with extreme scepticism. Dear Doctors, Pharmacists aren't stupider than you just because you trained for 3 or 5 years more.
Also, how about all the drug companies that give you freebies and what not? Stop being so up yourselves and get back to treating patients properly! Emma The issue is not about doctors egos. Blackmores has no interest in the health of patients only their bottom line - that should be a concern to everybody not just doctors. Why not any other of the numerous companies that provide "complimentary" products.
Why not recomend eg Coenzyme Q10 and then let the punter sorry patient choose between the different brands?? It won't put their health at risk. It will only put their money at risk. No-one will look after their money but themselves.
It'll put their privacy at risk too when the pharmacist asks in a loud voice if they want a certain supplement related to whatever condition they suffer from. If the computer was also suggesting a whole bunch of other also-suitable complimentary snake-oils from other manufacturers, then that wouldn't be so bad. But because it JUST blackmore's that's getting suggested, then it sounds like a pure money-grab and even as low as cash-for-comments. It always fascinates me how people passionately defend the 'scientifically proven' chemically based medicines and deride anything more natural.
Have you checked who actually does and funds the studies which are then simply used by government to 'approve' a medicine? Many if not most of these studies are conducted, if not at least funded, by the pharmaceutical companies themselves. And you really think you're going to get independent results from an industry the pharmaceutical industry that makes billions out of calling symptoms illnesses and thus justifying the treatment of symptoms rather then the actual problems, and who's business case is about ensuring people think they are ill so as to offer further treatment which stimulates further income So, you trust them enough to take something that has even less protection than prescription medication?
The AMA is having a go at the Pharmacy guild, what a joke, both doctors and pharmacists take what is considered normal in the industry for performance based selling, they get wined and dined, holidayed and golfed, all over the world at the expense of the drug companies and the drug wholesalers under the guise of conferences. It's all kept above board and made to look like professional meetings, but anyone with a friend who's a pharmacist or doctor will know they got the trip or the conference because of the amount of a particular drug they prescribed or dispensed, this is especially true with the generic drug market, when a new generic brand comes on the market and your pharmacist asks will you take the xxxxxxx generic, he's on a quota.
My kids have stopped coughing whereas if you give them Chemist cough medicine it does nothing. I suggest you have a good read of how they are made. Firstly, in the interests of disclosure and honesty - I am a registered pharmacist. Now for the broadside: When a script is issued from a doctor, there is a reasonable chance that some degree of recommendation has played a part in that decision, often based on the briefing produced by a representative of that manufacturer.
I find it difficult to understand the AMAs outrage as their practitioners often rely on information from representatives to assist with day-to-day decisions on patient treatment. Pharmacists DO have some commonsense and will only recommend additional products if it is a safe, b appropriate and c affordable.
Not everyone is satisfied with taking remedies supplied on prescription and indeed some customers do wish to either supplement their treatment or perhaps shock, horror It is their right to do so and this information should be available to them. At the end of the day, all the customer has to say is "no thanks" 5 If a brand had to be endorsed, Blackmore's would probably be one of a few brands I would trust in my work as well as a couple of others who are not the subject of discussion here. Scientifically speaking, there's a decent amount of active ingredients per serve and they DO have a biological basis of action.
Another leading supplement is often heavily promoted but barely rates a mention against some of the better-made products in terms of quality and content. I for one, read the label and make a decision before launching the sales spiel. The AMA would be well advised to examine the behaviour of its own practitioners with regard to this issue before embarking on a grandstanding exercise at the expense of their usual soft target.
Remember, medicine has a business element to it as well and is therefore open to input and manipulation from several parties, just as pharmacy is. The secret is to find a practitioner who is open enough to consider the various options out there and balanced enough to make the right call. You say that this is a matter of choice, but under this arrangement, filling a prescription for Medicine A will lead to being recommended Supplement B by a pharmacist. Many people will simply accept this advice in good faith, and that is not the same as making an informed decision.
How many people, given the opportunity to make an informed decision, would choose to spend their money on a supplement or medicine that has not even been tested for efficacy? How many people would actively choose to buy a product that only has a 1 in 10 chance of complying with TGA regulations?
How many people would choose a product that has a 1 in 3 chance of being cancelled when properly reviewed? I highly doubt that the pharmacist will be presenting it to consumers in those terms.
I know informed choice when I see it, and this isnt it. Upfront - I'm a Doctor I do not have a problem with Pharmacist providing information - it is essential. Where will it lead? Would you allow sponsorship from Carlton United Breweries? Clearly, the medical profession is very financially self-interested. But doctors do not, contrary to what you imply, profit from prescribing. Doctors are paid by patients and the government for the services they provide in consultations.
It is misleading to suggest that doctors profit from patients filling prescriptions. Only one health professional makes a profit from selling medicines. Any guesses as to who that is? The fact is that if a particular medicine precipitated the need for additional medicines or supplements, the prescribing doctor would probably have said as much.
Will you be telling consumers that the 'supplement' you're saying they need hasn't actually been tested by the regulator? Will you be telling them that the Blackmores 'supplement' in question can legally make therapeutic claims without having the evidence reviewed by the TGA or anybody else? Will you tell them that by combining supplements and complementary medicines with their prescription medication, they significantly increase their risk of an adverse event? Will you be telling them that their prescribing doctor didn't see fit to recommend this product at all?
If you don't want to share this information with the patient, that's fine. But as far as I'm concerned, you can shut the hell up about informed choice. Well, here's a little newsflash for you: You may think that your sugar daddies at Blackmores are perfectly lovely chaps, but that does not mean that you should use your time with your patients to upsell them products that they simply don't need.
Finally, considering your confidence in the 'biological basis' of these products, you might like to review this report from the Australian National Audit Office. It analyses the results of compliance reviews on the products sold by Blackmores and others: More than half required corrective action.
Only 1 in 10 actually stood up to the scrutiny. Will you be sharing this information with your patients, too? Roll on eDispensing, I say. Cut the pharmacist out all altogether. So many times I had to resist the prescription handed to me by my doctors on any slight abnormality of blood test results. It seems to be a habit of doctors to reach for their prescription pad on any symptom they encounter. So much so that makes suspect some sort of kickback is involved.
The last prescription drug my GP tried to give me was medicine to lower my cholesterol which I achieved simply by changing part of my diet. I build databases for doctors The doctors are all concerned about they can reduce their patient face-to-face time to take advantage of the Medicare rebates they can generate using Allied Health professionals they employ.
Blackmores were concerned with collating studies of the scientific aspects of their products. Doctors do not want to acknowledge any benefits from vitamin therapies It threatens the status quo of their income streams They see a threat to the pharmaceutical industry as a threat to their own community and financial standing.
A long time before there were doctors It is about time doctors recognised they are being replaced by technology LOL and that the goal of the pharmaceutical industry is to cut out the middle man.
Their loyalty to that industry is not viable in the long term and will be their demise. Contrast this to mainstream medicine which is largely evidence based. Indeed the curricula of our fine medical schools and colleges mandate it. Blind obedience to Big Pharma and a conventional medical system too dependent on surgery and technology has inflicted more harm than good on the U.
Look at the current damage done by over-prescribing antibiotics. Studies have learned that too much antibiotics cause trans-generational permanent DNA damage. The epidemic of trans-mutated bacterial infection and parasites that invade and infest the digestive tract in particular killing good bacteria and spread to other internal organs have become highly resistive to overuse of antibiotics.
Big Pharma and doctors know all this yet they are responsible for antibiotic overconsumption by uninformed Americans. Then look at what we are now learning about Big Pharma vaccines and the wanton reckless endangerment of children and pregnant mothers with toxic levels of mercury causing increased rates of autism, brain damage and even death. Instead of preventing and decreasing illness, vaccines too often have had the opposite effect, exponentially increasing illness, causing irreversible damage and even death to thousands of unsuspecting victims mostly living in Third World nations.
Starting in the final year of med school, Big Pharma insidiously hones in on young med students, seductively wining and dining prospective physicians, showering them with money in the form of educational handouts, gifts, trips and perks galore to recruit its legions of loyal, thoroughly indoctrinated drug peddlers around the world.
Big Pharma also invests more dollars into advertising than any other industry in America, transmitting its seductively deceptive message direct to its consumers, explicitly giving them marching orders to request specific drugs from their doctors. Their message — pleasure, relief, peace of mind, joy, love and happiness are all just a pill away. Of course they pay big bucks for slick ad marketing campaigns that shrewdly target the oldsters most apt to suffer health problems in addition to being virtually the only Americans left still watching the nightly network news.
Prozac became the biggest drug sold until it was learned that it caused so many people to kill themselves or others, especially adolescents.
Then Eli Lilly deceptively repackaged and relabeled it under the less threatening name Sarafem at a much higher price tailored to target unsuspecting women seeking relief from menstrual pain. Like Prozac as another Selective Serotonin Reuptake Inhibitor antidepressant, Paxil was suddenly repackaged as the cure-all for shyness under the guise of treating social anxiety. This most prevalent industry pattern of reusing the same old drugs all dressed up with new custom designed names for new purposes on new custom designed populations for yet more price gouging is nothing less than resorting to a predatory practice of criminal false advertising.
Perhaps as sinister as any aspect of the drug business is how Big Pharma has completely taken over the FDA. With complete autonomy and control, now pharmaceutical companies knowingly market drugs that carry high risk dangers for consumers. But because they so tightly control its supposed regulatory gatekeeper, drugs are commonly mass marketed and before the evidence of potential harm becomes overwhelming, by design when the slow bureaucratic wheels turn issuing a drug recall, billions in profit have already been unscrupulously reaped at the deadly expense of its victims.
Additionally, doctors, pharmacists and patients rarely even hear about important recalls due to dangerous side effects or contamination. This withholding the truth from the professionals and public consumers is yet more evidence that Big Pharma protects its profits more than people.
Research outcomes only need to show that the drug outperforms a placebo, not other older drugs already available on the market that have proven to be effective at lower cost. Similar to shady personnel moving seamlessly in and out of governmental public service to think tanks to universities to private law to corporations to lobbyists, the same applies to heads of the FDA moving to and from Big Pharma. Unfortunately this is how our government has been taken over by special interests.
